Business Statement of a Sports Expert or Professional Athlete
Notice on the issuance of a statement of business of a sports expert or professional athlete
The Ministry of Tourism and Sport of the Slovak Republic (hereinafter referred to as the "Ministry") establishes the procedure for issuing an extract from the Register of Natural Persons in Sport (hereinafter referred to as the "Extract"), which serves as a confirmation of the business of a sports expert or professional athlete pursuant to Act No. 440/2015 Coll. on Sport and on Amendments to Certain Acts (hereinafter referred to as the "Sports Act").
The extract is issued by the Ministry, which is the administrator and operator of the Sports Information System (hereinafter referred to as the "ISS") pursuant to Section 58 (l) of the Sports Act. This extract contains data on a natural person who is registered as an entrepreneur in the field of sport, namely as a sports expert or professional athlete under the provisions of the Sports Act.
The applicant submits an application for the issuance of an extract to the Ministry via the Central Public Administration Portal of the Slovak Republic (hereinafter referred to as "UPVS") to the Ministry's electronic mailbox via the Catalogue of UPVS Services (service provider – Ministry of Tourism and Sport of the Slovak Republic).
Note: To submit an application, it is necessary to have an electronic mailbox for electronic communication between citizens and entrepreneurs and public authorities. It is also possible to authorize another natural person who has set up an electronic mailbox for electronic communication between citizens and entrepreneurs with public authorities. This authorisation must contain an officially certified (certified) signature. In the subject of the electronic document, it is necessary to state: "Application for an extract from the ISS – business".
In the text of the electronic document, the applicant shall state:
- Business name,
- place of business;
- Company registration number,
name and surname,birth or identification number,permanent or temporary residence;Contact details (email address and phone number).
The request for an extract must be accompanied by a completed and signed consent to the processing of personal data pursuant to Act No. 18/2018 Coll. on Personal Data Protection.
If the application does not contain the above data, the Ministry will invite the applicant to complete it.
If the applicant does not complete the application within the set deadline, the Ministry will not be able to process it.
The request for an extract will be processed within 30 calendar days.
The validity of the issued extract is 90 calendar days from its issuance by the Ministry.
Each applicant is required to follow the above procedure.
The Ministry will continuously update this notice according to the applicable legislative changes.
By sending a request for an extract, the applicant confirms that he or she has become familiar with the protection of the controller's personal data published on the Ministry's website .
